Before appliances are placed we will have a financial consultation with the responsible party. It is a policy of our office to have one contract per patient. We do NOT offer split contracts.
We will arrange a contract that will allow you to make scheduled monthly payments. A payment booklet will be provided to aid you in making your scheduled monthly payments. We expect you to pay the monthly payment on the scheduled due date. No interest will be charged on this contract payment plan. The account must be kept current so that there will be no disruption of treatment.
After the records fee is paid we offer the following discounts for payment in FULL at the time contract is signed: 10% for payment with cash or check, 5% for payment with credit card.
Insurance
We have found that many of our patients have benefited from orthodontic insurance. We will file any orthodontic insurance for you. Insurance benefits will be paid directly to the cardholder. We have found it most beneficial for the cardholder to assign the checks over to Dr. Ratzlaff and then the benefit will be credited to the appropriate account.
